-호흡에 따른 피로 및 호흡 효율 개선
-흉곽 주변 보조 호흡근의 사용 감소 및 횡격막 운동 증가
 
※운동 방법 & 강도
-눕거나 앉아서 편안한 자세를 취한다
-한 손은 가슴, 한 손은 배에 올린다
-흡기 시 가슴은 움직이지 않고, 배가 움직이는 것을 느끼며 호흡한다
-처음에는 5~10분 동안 하루 3회 실시하고, 점차 시간을 늘린다
-가장 편한 자세에서 시작해 누운 자세, 앉은 자세, 선 자세, 보행하면서 호흡할 수 있도록 훈련한다
 
② 오므린 입술 호흡법
-입안에서 공기 압력을 높이고, 기도로 전달
-소기도의 폐쇄와 가래 축적 예방
-숨을 내쉬는 시간을 연장시켜서 호흡 수 감소
-안정 시 폐 용적을 늘려서 운동 능력 향상
 
※ 운동 방법 & 강도
-목과 어깨 근육이 이완된 상태로 천천히 코를 통해 흡입한다
-내쉴 때는 입술을 둥글게 모아서 촛불을 불 듯 천천히 내뱉는다
-들숨과 날숨 비율이 1대 2가 되도록 한다
-이완된 상태에서 천천히 길게 호흡하고, 억지로 내쉬는 것은 피한다

‘앞으로 기대기’
 
-팔로 몸을 지탱하면서 상체를 앞으로 기울인다
-복부근육을 이완시켜서 횡격막이 쉽게 아래로 내려가도록 돕는다
-팔을 고정시켜서 흉곽 주변 호흡 보조근이 효과적으로 작용하도록 한다
-의자에 앉아 팔꿈치를 받치고 상체를 앞으로 기울여서 천천히 숨을 쉰다

※ COPD 환자의 가래 배출에 도움이 되는 방법

① 머리를 약간 숙이고 의자에 기대어 앉는다

② 코로 천천히 깊게 숨을 들이마신 다음 ‘성문’을 닫고 3초 동안 숨을 멈춘다

(성문은 후두부에 있는 발성장치로, 목젖 주변 부위다)

③ 흉부와 복부의 압력을 높인 후 성문을 열면서 처음에는 가볍게 기침을 한다

④ 이후 세게 기침을 해서 가래를 배출한다

⑤ 가래가 깨끗하게 배출될 때까지 몇 차례 반복한다

① 유산소 운동

-COPD 환자의 호흡재활 중 가장 핵심

 

※ 운동 방법 & 강도

-5~10분 동안 가벼운 스트레칭 등 준비 운동을 한다

-20~60분 동안 걷기, 실내‧외 자전거 타기, 수영 등 유산소 운동을 한다

-중증 COPD 환자는 수압으로 호흡곤란이 심해질 수 있는 수영을 피한다

-한 번에 하기 힘들면 5분 운동을 6번 나눠서 총 30분 한다

-주 3일 이상하고, 운동 중 산소포화도는 90% 이상 유지한다

-운동 강도는 최대의 약 60%인 약간 숨이 찬 정도가 좋다

 

② 근력 운동

-엉덩이‧허벅지‧어깨 등 팔‧다리의 큰 근육 중심으로 진행

  

※ 운동 강도

-한 번에 10번 정도 들 수 있는 무게로 2~3세트 진행한다

-한 세트마다 휴식 시간은 2분 미만으로 한다

Ⅰ. 팔 근력 운동

 

※ 벽 짚고 팔굽혀펴기

-벽에서 30cm 정도 떨어져 선다

-발을 어깨너비로 벌린다

-손을 어깨 높이에서 어깨너비로 벽을 짚는다

-코가 벽에 닿을 듯 말 듯 한 자세까지 천천히 내려갔다 올라온다

-통증이 있으면 벽 쪽으로 좀 더 다가가고 손 간격을 좁혀서 한다

 

Ⅱ. 다리 근력 운동

 

※ 벽 이용 스쿼트

-선 자세에서 등을 벽에 붙인다

-천천히 무릎을 구부렸다 편다

-무릎은 90도 이상 구부리지 않는다

-10번 반복해도 힘들지 않으면 한쪽 다리를 들고 한다

-벽을 이용하지 못하면 책상‧의자를 잡고 한다

 

※ 앉아서 다리 들기

-의자에 앉는다

-한쪽 다리 무릎을 펴서 허벅지에 힘을 주며 10까지 센다

-무릎을 구부려 다리를 내리고, 2~3초 휴식 한다

-반대쪽 다리도 같은 동작을 한다

-강도가 약하다고 느껴지면 발목에 모래주머니를 달고 한다

 

※ 누워서 엉덩이 들기

-누운 상태에서 양쪽 무릎을 세운다

-양팔은 몸통 옆으로 약간 벌려서 자연스럽게 놓는다

-엉덩이를 들어서 10까지 천천히 센 후 엉덩이를 내린다

 

※ 계단 오르기

-계단을 오를 땐 허리를 꼿꼿하게 세운다

-발 모양은 11자를 유지한다

-두 발의 간격은 주먹 하나가 적당하다

-5분이면 아파트 계단 기준 15~20층 정도 오를 수 있다

-점점 층수를 늘려서 하루에 60층 이상 오르면 좋다

 

③ 유연성 운동

-만성 폐질환자의 흉곽 유연성 스트레칭

 

※ 어깨와 팔을 넓게 벌리며 심호흡한다

※ 상체를 좌우로 기울이며 심호흡한다
